Description Dorsal midbrain syndrome (DMS) is a supranuclear vertical gaze palsy secondary to lesions damaging the vertical gaze center in the posterior commissure of the dorsal midbrain. The syndrome is characterized by the following: (1) light-near dissociation,(2) paralysis of conjugate upgaze, and (3) convergence-retraction nystagmus during attempted upward saccades. Lid retraction also called Collier's sign in primary position may also be present.
