Description Bilateral, simultaneous, painless vision loss after pituitary adenoma surgery is rare. While iatrogenic causes are most common, failure of cerebral autoregulation may lead to breakdown in blood brain barrier leading to vasogenic edema affecting the occipital lobes, a clinical entity named posterior reversible encephalopathy syndrome(PRES). We report a rare case of PRES, five days after an uneventful transsphenoidal surgery for pituitary adenoma
