Description "Childhood immunizations have been proven as an effective way to prevent many devastating illnesses in children and adults. However, often children are behind on receiving all necessary immunizations. In an effort to fully immunize children, the Redwood Internal Medicine/ Pediatrics team was chosen as a pilot site to outreach to children under the age of six who are either coming due for immunizations or who are overdue for immunizations. Outreach efforts were targeted as a step towards recognition by the National Committee for Quality Assurance as a Patient Centered Medical Home. "
