Description The University of Utah Community Clinics has had a long standing goal to achieve control of our hypertension patients. Our target population for this project is patients ages 18+ who have been diagnosed with hypertension, with the control goal of blood pressure readings less than or equal to 140/90 for routine blood pressure readings and135/85 for home or AOBP readings. The number of providers meeting goal increased six percent. Overall control rate for all Community Clinics patients also increased during this time. However, barriers still exist and further work needs to be done.
