Description Giant cell arteritis (GCA) is an inflammatory vasculitis affecting large- and medium-sized arteries. The most common ocular manifestation is arteritic anterior ischemic optic neuropathy (AAION). MRI often shows enhancement of the extracranial and ophthalmic arteries; enhancement of the optic nerve sheath is very uncommon (Geiger et al., 2009). High dose steroids reduce the likelihood of fellow eye involvement, however, deterioration of vision continues in a subset of patients despite prompt treatment (Danesh-Meyer et al., 2005). Recovery of visual acuity occurs in a minority of patients (Foorozan et al., 2003).
