Description Neuromyelitis optica (NMO) is an autoimmune astrocytopathy caused by antibodies to the astrocyte water channel aquaporin 4 (AQP4). Cerebrospinal fluid (CSF) anti-AQP4 antibodies (AQP4-IgG) in NMO patients are the product of both passive diffusion and local autoantibody production. How AQP4-IgG and antibody producing cells (APCs) gain access to the CNS in NMO remain unclear.
