Description Acquired hyperopia with choroidal folds may occur with intracranial hypertension, occasionally prior to onset of papilledema [1,2,3]. Most reports are in patients with idiopathic intracranial hypertension, with only rare reports due to secondary increased intracranial pressure (ICP) [3,4,5]. We present a case of acquired hyperopia followed by progressively worsening headaches due to secondary intracranial hypertension from a complex right transverse sinus dural arteriovenous fistula (DAVF) found to have choroidal folds and papilledema. Hyperopia and choroidal folds should be recognized as potential early features of increased ICP, and this case highlights the need for a thorough work-up for all possible etiologies.
