| History | A 33-year old female at 30 weeks of gestation admitted to hospital after falling at home. Her hemoglobin level on admission was 11.6 g/dl. She was discharged after a period of observation, then readmitted 12 hours later with abdominal pain, dizziness, and vomiting. Her hemoglobin level on readmission was 10.0 g/dl. Fetal distress was detected and a healthy infant was delivered by emergency cesarean section at which time abruption of the placenta was discovered. Uncontrolled bleeding from the uterus ensued. An elevated D-dimer level, decreasing platelet count, and falling serum fibrinogen levels confirmed disseminated intravascular coagulation. The patient's hemoglobin level fell to 4.0 g/dl at which time blood replacement was begun and an emergency hysterectomy was performed. Post-operative: blurred vision with difficulty distinguishing colors. |
