Description Cotton wool spots (CWS) refer to areas of retinal whitening in the retinal nerve fibre layer and can best be regarded as boundary sentinels of inner retinal ischemia. The discovery of even a single CWS should prompt a workup for underlying associated systemic diseases. We present 4 patients with a single CWS associated with migraine and a negative exhaustive workup.
