Description Orbital apex lesions present diagnostic and therapeutic challenges. The following case exemplifies the risks and benefits of observation, steroid trial, and biopsy in the orbital apex. Furthermore, it illustrates the trans-sphenoidal endoscopic approach to the apex, which may sometimes be advantageous to the trans-orbital approach when tissue diagnosis is necessary.
