Description Cranial nerve palsy can occur in multiple myeloma from direct infiltration, secondary ischemia, or indirect effects of chemotherapy. We report a case of an acute, neurologically isolated pupil sparing third nerve palsy resulting from infiltration of multiple myeloma into the superior lateral cavernous sinus.
