Description This is an unusual case report of a fifteen year old white male who first presented on 5-15-78 with a history of right upper eyelid drooping of one week duration. His vision was 20/20 in each eye and he was orthophoric in eyes front, but showed under action of the left inferior rectus muscle with diplopia looking down and left only. He had 3 mm right ptosis and the right pupil was 4 mm while the left pupil was 4 1/2 mm. Fundus examination and visual fields were normal. There was no exophthalmos.
