Description Vertical saccadic palsies are reported in lesions that affect the midbrain structures involved in the control of vertical gaze, mainly the rostral interstitial nucleus of the medial longitudinal fasciculus (riMLF), the interstitial nucleus of Cajal (INC), and the posterior commissure (PC). According to previous studies, combined upward and downward vertical saccadic palsies require either a bilateral intra-axial lesion involving the riMLF or a unilateral intra or extra-axial lesion involving the posterior commissure.
