Description Neurotoxicity is a known complication associated with calcineurin inhibitors with wide clinical presentations reported. However, imaging characteristics and pathogenesis remain unclear. We describe the clinical presentation, imaging characteristics and pathological findings of four post-solid organ transplant patients on maintenance immunosuppression with calcineurin inhibitors who presented with symptoms mimicking optic neuritis with a progressive and fatal course.
