Description Superior ophthalmic vein thrombosis (SOVT) is a rare clinical entity with potentially devastating consequences. SOVT is commonly unilateral and the average age of presentation is 52. Infection, inflammation, trauma, a hypercoagulable state, artero-venous fistula can predispose a patient to develop thrombosis. A third of cases were due to a septic etiology in a retrospective review of 60 cases. When secondary to infection, the pathophysiology is due to thrombophlebitis causing venous stasis and/or direct invasion of the vessel by the pathogen resulting in a septic thrombus. Herein, we present a case of SOVT in the setting of preseptal cellulitis in a young boy leading to the discovery of a rare hypercoagulable state.
