Description A 32-year-old male was admitted to the clinic after being hit by a gum-end spear at the left eye. At the time of the injury he was wearing scleral contact lenses (SCL), not found. The eyeball seemed intact, but was blocked in a downward position. It was a diagnostic challenge to decide whether the incomitant diplopia was due to the SCL behind the globe, muscle entrapment, retrobulbar hematology or direct muscle/nerve injury.
