Description Diagnosis of tonic pupil (TP) relies in part upon demonstratlon of 'denervation supersensitivity' of the iris sphincter. Most studies on TP have used the fellow eye as an internal control. TP Is a slowly progressive disorder that becomes bilateral in as many as 50% of patients. It is questionable whether the fellow eye can be accepted as a normal control. Also, some patients present with bilateral TP, and no internal control eye exists in these patients.
