Description Hemolytic uremic syndrome (HUS), most often triggered by infection with Shiga toxin producing Escherichia Coli, is characterized by microangiopathic hemolytic anemia, thrombocytopenia, and acute kidney injury. Less often, atypical HUS (aHUS) can result from dysregulation of the alternative complement pathway due to genetic mutations.1 We report a case of bilateral optic disc swelling in a patient with aHUS attributed to a mutation in thrombomodulin, which encodes a vascular endothelial glycoprotein.
