Description Lumbar puncture is performed frequently in patients with idiopathic intracranial hypertension. Cerebellar tonsillar descent is not uncommon, but frank herniation (coning) is extremely rare.1 We are aware of only one previous case reported in the literature.2 We describe a case in which the patient coned following a lumbar puncture, necessitating acute neurosurgical intervention.
