Description An 83-year-old gentleman presented with acute bilateral visual loss. Visual acuity was NLP OD and 20/50 OS. His motility was full and a relative afferent pupillary defect was noted OD. Goldmann visual field testing OS revealed an inferior altitudinal defect with a temporal wedge scotoma. His fundus exam was significant for moderate pallid disc edema OU. A stat ESR was 60 mm/hr. An MRI of the brain and orbits with gadolinium demonstrated marked bilateral enhancement of the optic nerve sheaths and adjacent orbital fat. The patient subsequently underwent biopsies of his optic nerve sheath OD and bilateral temporal arteries.
