Description Bilateral optic disc swelling with preserved visual function prompts a list of differential diagnoses. In an older patient the diagnostic process may be quite cumbersome, and unless meticulous observations are undertaken, a delayed diagnosis of a systemic disease may ensue. POEMS syndrome (Polyneuropathy, Organomegaly, Endocrinopathy, Monoclonal gammopathy, and Skin changes) is a rare multisystemic disease that occurs in the setting of a plasma cell dyscrasia.
