Description |
This is an illustrated guide to the clinical diagnosis of optic nerve hypoplasia. Optic nerve hypoplasia (ONH) is the most common congenital optic nerve anomaly, with an estimated incidence of 1 in 2287 live births. It may present unilaterally or bilaterally. It is seen in isolation or in association with several ocular and systemic conditions (Table 1). Clinical features include: a small optic nerve (reduced diameter); peripapillary halo ("double ring sign"); vision that can range from normal to severely reduced; the presence of nystagmus or strabismus. ONH has been associated with several brain abnormalities, including hydrocephalus, agenesis or hypoplasia of the septum pellucidum and corpus callosum and abnormalities of the pituitary gland. The association of ONH with midline brain findings has been referred to as "Septo-optic dysplasia" and "de Morsier syndrome". All patients with optic nerve hypoplasia should receive an endocrinological evaluation and undergo a MRI brain. |
