Description Isolated bilateral abducens nerve palsy is uncommon and has a broad differential diagnosis. This includes infectious meningitis/encephalitis, intracranial hemorrhage, cerebrovascular accidents, elevated intracranial pressure secondary to neoplasm, inflammatory conditions, or pseudotumor cerebri, demyelinating disease, vasculitis, and other vasculopathies including aneurysm, carotid-cavernous fistula and arteriovenous malformations. (1) We present a case of a complex arteriovenous malformation causing isolated bilateral abducens nerve palsy.
