Slavin, Long Island Jewish Medical Center, New Hyde Park, NY 11042.] A patient with limited elevation of her right eye on adduction and minimal proptosis was found to have presumed metastasis to the superior oblique muscle from her breast carcinoma. Dagi, Center for Sight, Georgetown University, 3800 Reservoir Road, N.W., Washington, DC 20007.] Seven patients with central nervous system disease (pituitary adenoma, head trauma, ArnoldChiari malformation, etc.) had spasm of the near reflex with minimal symptomatic disability. The authors contrast these patients with probable organically caused spasm to the more common functional patients with spasm of the near reflex. LYIl A. Krachmer, Department of Ophthalmology, University of Iowa Hospitals and Clinics, Iowa City, IA 52242.] A 34-year-old man with a 5 year history of coldinduced decreased vision in the right eye is discussed. Examination showed right fifth nerve paresis and, following placing the patient in a cold room for 2 h, his right cornea became edematous and vision was degraded. Computerized tomographic scan demonstrated a calcific lesion in the right middle temporal fossa which was totally resected and found to be a meningioma. Postoperatively, his corneal edema in the cold continued. The authors postulate faulty innervation of the corneal endothelial pump because of the meningioma but readily admit that there is no experimental data to confirm their hypothesis. Lyn A. Nikoskelainen, Department of Ophthalmology, Turku University Central Hospital (TYKS), 20520 Turku, Finland.] This important study of four pedigrees with several generations each in Finland helps to make good sense of a previously baffling inheritance pattern. The authors show that all offspring of women carriers, male and female, have some evidence ?f the disease albeit most often only a microanglOpathy on retinal examination which is asymptomatic. Moreover, there are cardiac conduction abnormalities which seem to be inherited with Leber's in these pedigrees. The authors contend ~hat. maternal inheritance through cytoplasmiC mitochondrial DNA is likely. Lyn A.
