History A 48-year old female with a 1-week history of loss of vision OD. Previous history significant for night sweats, fever, myalgia, fatigue, loss of appetite and six pound weight loss she associated with a trip to Mexico where she was treated with indomethacin for a non-pruritic skin rash on both arms.
Pathology Gram-positive bacteria on aortic leaflets
Disease/Diagnosis Bacterial endocarditis
Clinical VA: 20/200 with eccentric gaze OD, 20/40 OS; IP: 12 OU; RAPD OD
Presenting Symptom Loss of vision OD
Neuroimaging MRI
Treatment IV anti-bacterial agents
