The "Passport to Infusion" is a paper checklist that clinic nurses use to ensure patient variables are addressed prior to the patient's arrival to the infusion room. The Passport was designed with input from all the outpatient clinics. Over a period of several months, the Passport was refined to include the necessary tasks the clinic nurse needs to complete before the patient can advance to the infusion room. The Passport was then trialed in one clinic to evaluate its effectiveness (June-August 2016). With the data compiled from this trial, changes were made to the Passport, staff input was analyzed and data collection methodology was evaluated. With the Passport form finalized, initiation to the other clinics commenced in September 2016. Continue to work closely with individual clinics/providers to improve Passport compliance. Provide education and support to further decrease the number of unsigned orders. Increase the percentage of completed orders for non-chemo treatments: Blood transfusions, Iron, IVIG, and hydration fluids/electrolytes. Improve compliance and evaluate process for patients whose doctor visit and infusion appointment are on different days. |
