Description Severe cocaine-induced midline destructive lesions (CIMDL) can cause orbital cellulitis and optic neuropathy. We present the first case of acute, painless vision loss in the setting of CIMDL with neuroimaging suggestive of posterior ischemic optic neuropathy (PION).
