History A 78-year old female with binocular horizontal diplopia, worse in right gaze and intermittent right frontal headache. Previous history significant for non-insulin-dependent diabetes mellitus, hypertension, hyperclosterolemia, arthritis, coronary artery disease and atrial fibrillation. Bilateral cataract extractions 5 years prior to presentation.
Pathology Right clival tumor composed of atypical plasma cells consistent with plasmacytosis; Myeloma involving femurs, humeri and T11 vertebral body
Disease/Diagnosis Multiple myeloma involving skull base and covernous sinus
Clinical VA: Best-corrected, 20/30 OD, 20/20 OS. Normal color vision OU
Presenting Symptom Diplopia; Headache
Neuroimaging MRI
Treatment Surgery; Antineoplastic agents; XRT
