History A 37-year old female with a history of hepatocellular carcinoma, status post liver transplant x 2, presented to the ED with a 1-day history of right-sided headache, right eye pain, photophobia and unsteady gait. Two days prior to presentation patient had been discharged following second transplant.
Pathology Subretinal lesion OD; Acute fungal cerebritis
Disease/Diagnosis Fungal infection with Scedosporium inflatum
Clinical VA: 20/20 OD, 20/20 OS
Presenting Symptom Right-sided headache; Right eye pain; Photophobia; Unsteady gait
Neuroimaging CT; MRI
Treatment Antifungal agents
