Hypertension is the most common condition seen in primary care and leads to myocardial infarction, stroke, renal failure, and death if not detected early and treated appropriately. We were unable to achieve our goal of 80% hypertension control. We feel that outreach to a single provider's patient panel was adequately validated. This validated process may now be expanded to include dedicated outreach time for multiple staff members. It is anticipated that this will lead to increased hypertension control or will allow insight as to the next best intervention. |
