History A 64-year old female with a 4-year history of transfusion-dependent myelodysplasia noted right facial pain radiating from the angle of the jaw to the temporal area. The pain was attributed to sinus disease and empirically treated with oral antibiotics. Four weeks later, she awoke with NLP OD. Previous history significant for surgically-treated colon cancer, coronary artery disease and splenectomy Tx prednisone, deferoxamine and cardizem.
Pathology Infarcts of basal ganglia, thalami, midbrain and pons
Disease/Diagnosis Deferoxamine-associated angioinvasive mucormycosis
Clinical VA:NLP OD, 20/25 OS;.S.. Amaurotic right pupil; Swollen right retina and optic nerve.
Presenting Symptom Facial pain; Loss of vision OD
Neuroimaging CT; MRI
Treatment Anti-bacterial agents; Anticoagulants
