History An 83-year old male with visual hallucinations and a change in personality. Previous history significant for surgically treat colonic neoplasm, MI, cataract extraction OD, III nerve palsy, diabetes mellitus, peripheral neuropathy and cardiomegaly with mitral regurgitation.
Pathology Cardiac hypertrophy; Atherosclerosis; Liver cirrhosis; nephrpsclerosis; Thalamic occulsion; Lesions within pars reticulata of the substansia nigra characterized by macrophagic infiltration.
Disease/Diagnosis Peduncular hallucinosis
Clinical VA: 20/40 OD, 20/25 OS; Bilateral soft macular drusen with normal optic discs.
Presenting Symptom Visual hallucinations; Personality change
Neuroimaging CT
Treatment Anticonvulsants; Antimanic agents
