History A 57-year old African-American male with transient visual obscurations brought on by exercise. Painless black vision associated with neck and lower back pain. Occipital headache, left facial numbness and bilateral leg weakness. Previous medical history significant for HIV, pneumonia, facial zoster, colitis, hepatitis C, seizure, pancreatitis, hypertension and diet-controlled diabetes mellitus.
Pathology Inflammatory eosinophilic leukocyte infiltration with destruction of elastic lamina of vessels.
Disease/Diagnosis Eosinophilc vasculitis
Clinical VA: 20/30 with amaurotic episode, 20/25 OS
Presenting Symptom Transient visual obscurations with amaurosis
Neuroimaging MRI
Treatment Steroids; Anti-HIV agents; Anticoagulants
