History A 54-year old female with blurred vision, mild confusion, diffuse weakness and shortness of breath. Previous history significant for bilateral lung transplantation for complications of acute respiratory distress syndrome followed aspiration pneumonia. Past ocular history significant for bilateral cataract extraction and left corneal transplant for toxic anterior segment syndrome.
Pathology Fibrin thrombi within capillaries and arterials and associated acute infarcts of the hypothalami, frontal lobes, temporal lobes and pituitary stalk consistent with calcineurin inhibitor toxicity.
Disease/Diagnosis Thrombotic microangiopathy related to calcineurin inhibitor toxicity
Clinical VA: 20/400 OD, 20/50 OS; Bilateral optic atrophy and bilateral visual field defects
Presenting Symptom Blurred vision; Diffuse weakness; Shortness of breath; Confusion
Neuroimaging MRI
Treatment Anti-infective agents; Plasmapheresis
