Description Indirect traumatic optic neuropathy (ITON) is a rare but important cause of vision loss in head trauma. Although optic nerve head pallor develops over time, confirming a diagnosis of ITON in the acute period can prove challenging given the normal appearance of the visual system. We describe the acute neuroradiographic appearance of a patient with profound vision loss from ITON, highlighting a role for magnetic resonance (MR) imaging in this context.
