Description The etiology of most isolated cranial nerve palsies is classically taught to have a micovascular origin. Given age and additional comorbities such as diabetes, hypertension, and hyperlipidemia the need for immediate neuroimaging in these patient remains controversial. This case presentation highlights the importance of early imaging even in a population with clear vascular risk factors.
