Description The most common degenerative condition affecting the extrapyramidal system is idiopathic Parkinsons disease (IPD), described in 1817 by James Parkinson in his monograph ""An Essay on the Shaking Palsy."" Slowness of emotional and voluntary movement, muscular rigidity, tremor, a pathologic striatal deficiency leading to a dopamine deficiency, and onset in middle and late life are characteristic of IPD. However, there are a number of disorders with similar features (i.e., parkinsonism).
