Description Internuclear ophthalmoplegia has been a presenting manifestation of numerous diseases including multiple sclerosis (commonly bilateral), brainstem infarction (commonly unilateral), infections, brainstem and fourth ventricular tumors, etc. We present a patient who presented with the above and on diagnostic work up was found to have evidence of progressive multifocal leukoencephalopathy involving the brainstem.
