Alternative payment models, increasingly complex chronic conditions and national quality initiatives such as Patient Centered Medical Home have necessitated care management services in primary care. Care management is a relatively new specialty and minimum qualifications and professional requirements have not been established, which leads to the use of less qualified individuals. Data to demonstrate return on investment has been elusive. Conclusion: Based on their educational preparation and experience, registered nurses possess the best skill set to carry out the maximum functions of care management that will ensure the most positive outcomes and results. |
