History A 68-year old hypertensive female with difficulty walking, dizziness and lethargy. Previous history significant for subacrachnoid hemorrhage which resulted in residual left hemiparesis.

Clinical Examination demonstrated patient oriented to person and place but not to time. Cheyne-Stokes respirations were noted, blood pressure: 130/90. Continuous, synchronous, fast downbeating eye movements (resembling downbeat nystagmus) were occasionally interrupted by larger amplitude rapid downward jerks with slow return to the primary position (ocular bobbing).
