Description A 31y/o white male was transferred to Wills Eye Hospital with a diagnosis of cellulitis secondary to sinusitis unresponsive to IV antibiotics and sinus drainage. His history was that of increasing conjunctival chemosis, proptosis, and continued loss of vision. At the time cf admission his visual acuity was CF at one foot. He had nine millimeters of proptosis. No pulsations or bruits were present, but there was a question of arterialization of the conjunctival vessels. The upper lid was swollen and erythematous. Extraocular motility was limited in all fields of gaze. The cornea was bedewed and the intraocular pressure was elevated to 28. Occasional cells and flare were present in the anterior chamber.
