Description |
At least 68% of people age 65 or older with diabetes die from heart disease. Diabetes is considered to be one of seven major controllable risk factors for cardiovascular disease. Of patients with type 2 diabetes between the ages of 40-75 years treated at the South Jordan Health Center during the study period: Over 50% did not meet the 2016 HEDIS measure for statin therapy; Majority are undertreated with statin intensity according to the ACC/AHA cholesterol guidelines. Primary care pharmacists will promote appropriate use of statins in patients with diabetes through consult and collaborative practice agreement, Provider education regarding recommended statin therapy. |
