Abstract Nonarteritic anterior ischemic optic neuropathy (NAION) has no proven treatment and typically causes some degree of irreversible visual loss. Preventing second eye involvement is a worthy goal, but there is uncertainty regarding the benefit of any preventive therapy. Two experts debate the role of aspirin in preventing fellow eye involvement in NAION.
