Description Hematopoietic stem cell transplantation (HSCT) is a successful treatment for many hematologic malignancies. Despite successes, its known risks include graft-versus-host disease (GVHD). Ocular manifestations occur in up to 90% of patients with chronic GVHD, most commonly affecting the ocular surface, though some cases involve the posterior segment. This unique case reveals a sole clinical exam finding of bilateral optic nerve atrophy in a patient not treated with cyclosporine A. This presentation is completely unique. Furthermore, this study provides objective data from electrophysiologic studies.
