Description There has been much controversy over the merits of intracranial versus infrasellar surgery for pituitary tumors, particularly chromophobe adenomas. Infrasellar approaches permit subtotal removal or minimal decompression of tumor. Total removal is difficult if not impossible. Although transfrontal craniotomy affords the surgeon a better view, total removal by the route is seldom accomplished because of the difficulty and hazard of removing remnants of tissue from the walls of the hypothalmus or the enlarged sella turcica.
