Description A 42-year-old male complained of episodic visual disturbance in his left field of gaze for three months, posterior neck pain radiating to the right shoulder and arm, vertigo, nausea, disorientation, numbness and clumsiness of the right upper limb. He denied diplopia or slurred speech. Past medical history is positive for Crohn's disease, status post colectomy and ileostomy. He is on Oipentum (olsalazine sodium). He denied any history of hypertension, diabetes or migraine. Family history is positive for mild migraines in his mother.
