Description The American Cancer Society estimates there will be 2.8 million breast cancer survivors in the United States in 2014. Because of the increased survival rates in breast cancer there are growing numbers of survivors who need comprehensive survivorship care for longer life spans. This long term care requires better communication between oncologists, patients, and primary care providers to manage specific needs and improve quality of life of survivors (American Cancer Society, 2014). There are many organizations who feel survivorship care plans (SCPs) will improve communication. The Coalition on Cancer requires implementation of a SCP at cancer centers by January 2015 to maintain accreditation. Prior research indicates providers and patients prefer SCPs be implemented in the Electronic medical record (EMR). The purpose of the project was to develop and implement the treatment summary and prevention portion of the breast cancer Survivorship Care Plan into the EMR, increasing provider compliance by addressing barriers and improving the quality of the SCP.  Objective 1: Participate in the Quality Improvement Project that will address and create survivorship care plans for use at a local cancer institute.  Objective 2: Identify components of the treatment summary and prevention portion of a breast cancer SCP, based on existing national recommendation's evidence.  Objective 3: Integrate surveillance and long-term follow-up plan sections of the breast cancer survivorship care plan into the EMR, Epic, on January 1, 2015.  Objective 4: Evaluate the use and effectiveness of the surveillance and long-term follow-up plan portion of the breast cancer survivorship care plan by surveying participating oncologists regarding ease or barriers to use.  Objective 5: Disseminate survey results to the Quality Improvement Committee at the local cancer institute to guide future improvements. The Quality Improvement Team and two content experts, a survivorship oncologist and an epic specialist, were used to develop and implement the SCP in the EMR. An electronic questionnaire through REDcap was sent to providers and used to a guide improvements. Five providers used the SCP and felt it may decreased time and does improve quality. Time remains the largest barrier. Increased use cannot be measured because there are not past records of SCPs. Education and support from administration is needed to increase use. No changes will be made to improve the SCP in the EMR at this time.
