Presenting Symptom Blurred vision OU
History A 68-year old male with 1-1/2 year history of painless blurred vision OU. Previous history significant for subacute paraparesis from spinal meningitis (subsequently resolved), ischemic optic neuropathy with residual central loss of vision OS. Remote history of a 78 pack-year of smoking.
Clinical VA: Best-corrected, 20/40 OD, 20/200 OS; Reduced low-contrast and color vision the left > right; Optic nerve atrophy OS worse than OD; Central scotoma
Neuroimaging MRI; PET/CT
Pathology Metastatic poorly-differentiated adenocarcinoma
Disease/Diagnosis Paraneoplastic optic neuropathy associated with adenocarcinoma of the lung/mediastinum
Treatment Gamma knife surgery; Antineoplastic agents; IV neuroprotective agents; IVIG
