History 76-year old male; five year history of muscle pain and arthralgia; four year history of PMR symptoms; gastrointestinal illness.
Pathology Intraconal mass which demonstrated heterogeneous enhancement and appeared to extend to the optic canal; an extensive necrotizing process with palisading histiocytes and multinucleate giant cells with an inflammatory infiltrate, including neutrophils, lymphocytes, and plasma cells
Disease/Diagnosis Necrotizing granulomatous inflammation of the orbit and sclera without systemic disease
Clinical 20/20-2 in the right side, light perception in the left side.
Presenting Symptom Ptosis; impaired eye movements; headache; forehead numbness
Neuroimaging CT; MRI
Treatment Low dose steroids; Mycophenolate
