Description Despite its advocation by healthcare personnel, incentive spirometry (IS) is not an evidence-based practice. The reason for this most probably stems from low adherence rates caused by forgetfulness, patients unable to access the device, or patients not using it effectively. The purpose of this quality improvement project was to create a sustainable, evidence-based plan to improve IS adherence rates so the long term effectiveness of IS in improving pulmonary outcomes and decreasing respiratory complications could be evaluated. To accomplish this goal, healthcare personnel implemented interventions utilizing a multidisciplinary approach, patient family members, and improved patient education strategies. Data was collected pre-and post-interventions via chart and room audits to determine the number of patients with IS, the number of IS sessions patients received, the location of the IS device, and the patient's ability to use it correctly. The interventions lead to a statistically significant improvement in the number of patients participating in IS and the number of IS sessions patients received. The location of the device within the patient's reach and their ability to use it correctly increased, but was not statistically significant. Standards of care in the unit changed to emphasize IS use, lending to the sustainability of the project. As IS adherence rates continue to improve, its effect on patient respiratory function can be evaluated.
